TL;DR
Not on its own. A healed below-knee amputation records that a leg was removed; it is not a diagnosis of peripheral vascular disease, and E11.51 needs one. Once the provider documents peripheral vascular or arterial disease, the classification links it to the diabetes and E11.51 applies: the provider does not have to call it diabetic. Until then, code what is documented, including the amputation itself (Z89.511), and ask about current vascular status.
01The note
Type 2 diabetes on metformin. Healed right below-knee amputation. Can I code diabetes with peripheral angiopathy?
The diabetes is documented and managed at this visit. The question is the vascular complication.
02The link is presumed. The disease is not.
ICD-10-CM presumes a link between diabetes and the conditions the classification joins to it with "with". Peripheral vascular disease is one of them: in the Alphabetic Index, "Disease, peripheral, vascular, in diabetes mellitus" sends you to "Diabetes, with, peripheral angiopathy", E11.51. So when PVD is documented, the provider does not need to write "diabetic". The link holds unless the provider says the two are unrelated.
What the presumption cannot supply is the PVD itself. Code assignment rests on the provider's statement that a condition exists, and an amputation is a procedure in the history, not that statement. A leg can be lost to trauma, an infection or an ulcer as well as to vascular disease. Reading gangrene into it is a second inference on top of the first.
03What to code now
The type 2 diabetes, as documented. And the amputation, as a status: Z89.511, acquired absence of right leg below knee. Status codes in category Z89 are for use when the amputation site has no complications, which "healed" describes.
04What to ask
Ask about the patient's vascular status now, not only why the leg was removed. An outpatient code needs a condition that exists and affects care at this visit, and querying for a diagnosis that would not meet that test is inappropriate. Even a diabetic cause for the old amputation would not show that vascular disease is present today.
Give the provider the facts as found and where they were found, and offer only options the record supports:
- Peripheral arterial disease (please specify)
- Other, please specify
- Unable to determine
Add a choice such as trauma only if something in the record points to it. The subject line the provider sees stays neutral, for example "CDI provider query: vascular status", never "Diabetic angiopathy query".
05What each answer means
- Peripheral arterial or vascular disease documented: E11.51, presumed diabetic unless the provider says otherwise. E11.52 only if gangrene is documented too.
- Another explanation: code what the provider documents.
- Unable to determine, or no answer: no vascular complication is coded. The diabetes and Z89.511 stand.
06What not to do
- Code E11.51, or E11.52 with gangrene, from the amputation alone.
- Ask "Was the amputation due to diabetes?" A yes/no question may confirm a link between two documented conditions, but it may not introduce a new diagnosis, and PVD is not documented here. A "yes" would not establish angiopathy either: an amputation related to diabetes can follow an ulcer or an infection.
- Ask the provider to call documented PVD "diabetic". The classification already links them.

Questions
Does the provider have to call the vascular disease "diabetic"?
No. Peripheral vascular disease in a patient with diabetes is linked by the classification, so documented PVD codes to E11.51 unless the provider states it is unrelated.
Does a history of amputation support a code on its own?
Yes, as a status: Z89.511 for a right below-knee amputation. It does not support a diabetic vascular complication; that needs the vascular disease documented.
Should I query for gangrene too?
Only if the record gives a reason to. Gangrene is a diagnosis the provider has to document; a healed amputation is not evidence of it.
Educational. It explains the guidelines; it is not a coding decision for a specific chart.
