V28 compliance intelligence at the point of documentation.
Neurona 28 validates coding decisions against documented clinical evidence under V28 rules. It is a workflow instrument — not a replacement for clinical judgment.
The Pocket Auditor is a clinical documentation review tool that analyzes medical records against CMS-HCC V28 coding rules. It identifies documentation gaps, validates diagnosis code accuracy, and generates audit-defensible output. Unlike general-purpose AI, it is evidence-gated — it will not suggest a code unless the clinical documentation explicitly supports it.
What it does
- Maps documented clinical findings to V28-compliant ICD-10 codes.
- Links every suggested code to the specific clinical input that supports it.
- Structures output with MEAT evidence chains (Monitor / Evaluate / Assess / Treat) for audit defensibility.
- Flags documentation gaps and generates non-leading physician queries.
- Processes bilingual (Spanish-English) clinical input natively.
What it does not do
- Does not diagnose patients or provide medical advice.
- Does not connect to or scrape Electronic Health Records.
- Does not infer conditions beyond what is documented.
- Does not fill documentation gaps with assumptions.
- Does not replace the professional's clinical judgment.
Neurona 28 is in private early access. The current version is an MVP focused on core V28 validation for a limited set of clinical conditions. Expect ongoing iteration, feedback loops, and transparent communication about what the tool can and cannot do at each stage.
All examples shown are synthetic. This tool is not medical advice and does not diagnose or treat conditions.
